Health and Wellness Assessment

Roger
Age: 55
Occupation: Roger has spent 20 years as a long-distance truck driver.
Lifestyle: Roger smokes 1 pack of cigarettes a day, eats mainly fast food and processed snacks. BMI = 30
Family history: Roger has a history of heart disease and high cholesterol.
Fitness plan: Roger only goes on occasional walks; he has no regular physical activity

Dianne
Age: 40
Occupation: Investment banker
Lifestyle: works 60+ hours per week sitting at her desk in a highly stressful job. She often takes her work home with her. She is not married and spends much of her free time at the office. Dianne eats a very healthy diet; however, she is constantly dieting. BMI = 19
Family history: depression and bi-polar disorder among family members. Friends of Diane have commented on her euphoric episodes along with deep lows. Dianne is having problems sleeping and has started using Xanax for help with sleep.
Fitness plan: Dianne is lucky to have a gym on her work site. She works out at least 1 hour 3 times weekly.

Janet
Age: 46
Occupation: Janet is a college professor whose husband has early onset Alzheimer’s disease. Janet spends all of her free time taking care of her husband. She has no social activities outside of her work and is constantly feeling overwhelmed and tired.
Lifestyle: Janet tries to eat a healthy diet; however, with her many duties as professor and caretaker, she eats whenever and wherever she can. BMI = 27
Family history: heart disease and mental illness
Fitness plan: Janet rides her bike to work; she works out 5 days a week for 20 minutes/workout.

Mildred
Age: 75
Occupation: widowed and retired
Lifestyle: Mildred lives alone. She has two daughters; however, they live out of state and she sees them only once or twice a year. Sometimes she goes an entire week without speaking to others. Mildred’s diet includes a mix of healthy food and prepared food and snacks. BMI = 23
Family history: heart disease, breast and colon cancer, high cholesterol, kidney disease. Mildred has not had a physical exam in five years and rarely goes to her doctor.
Fitness plan: Mildred’s physical activity is limited to occasional walks totaling less than 2 miles per week.
